Non-Surgical Cancer Treatments

One of the most advanced points medicine has reached is that cancer can now be treated not only surgically, but also through minimally invasive, targeted, and tissue-preserving methods without surgery. These innovative approaches that serve as alternatives to surgery provide patients with a more comfortable recovery process both physically and psychologically.

What is Non-Surgical Cancer Treatment?

Non-surgical cancer treatments are based on the principle of destroying cancerous tissues or tumors without removing them from the body, using heating, freezing, burning, or chemical ablation methods. These procedures are performed using technologies such as radiofrequency, microwave, cryoablation, and ethanol ablation.
The goal is to ensure complete tumor destruction while preserving healthy tissues and allowing the patient to return to daily life as quickly as possible.

These methods are applied with great success, particularly in liver, lung, kidney, breast, bone, thyroid, soft tissue, and lymph node metastases.

When Are They Preferred?

  • If tumor size is smaller than 5 cm

  • If tumor number is limited (single or few lesions)

  • If surgical risk is high or general anesthesia is not appropriate

  • If there is recurrent tumor after surgery

  • If the patient has aesthetic concerns or desires rapid recovery

In these situations, non-surgical cancer treatments can be applied either alone or in combination with chemotherapy, radiotherapy, or immunotherapy.

Methods Used

1. Radiofrequency Ablation (RFA)

RFA is one of the longest-established and most successful methods in cancer treatment.
Under ultrasound or CT guidance, a thin needle is inserted into the tumor tissue.
Radiofrequency energy raises the temperature inside cells to 90-100°C, destroying cancer cells.

Advantages:

  • Local anesthesia is sufficient; general anesthesia is not required.

  • It can be applied without damaging blood vessels, bile ducts, or nerve tissue.

  • Complete tumor destruction can be achieved in a single session.

  • There are no surgical incisions or sutures.

  • Patients can be discharged the same day.

Applications:
Liver tumors (HCC, metastases), lung and kidney tumors.


2. Microwave Ablation (MWA)

Microwave energy produces rapid heat increase in tissue, destroying tumor cells.
Because it can reach higher temperatures compared to RFA, it is preferred for larger tumors.

Advantages:

  • Procedure time is shorter.

  • There is no heat loss around blood vessels (heat sink effect is minimal).

  • More effective results are achieved in larger tumors.

Applications:
Liver, kidney, lung, and bone metastases.


3. Cryoablation (Freezing Treatment)

In cryoablation, liquid argon is used to freeze tumor tissue to temperatures between -40°C and -80°C.
Cold causes cell membranes to rupture, resulting in cell death.

Advantages:

  • Tumor shape and borders are clearly visualized during the procedure.

  • It is painless; local anesthesia is sufficient.

  • It is particularly effective for soft tissue, breast, and bone tumors.

Applications:
Breast, kidney, bone, lung, and soft tissue tumors.


4. Ethanol (Alcohol) Ablation

For small-sized tumors, pure ethanol is injected directly into the tumor to damage cell membranes and cause tissue necrosis.
It is a simple, economical, and highly effective method.

Applications:
Thyroid, liver, and some lymph node metastases.


5. Transarterial Treatments (TACE, TARE)

These methods are generally applied for liver tumors.
A catheter is introduced into the artery supplying the tumor, and chemotherapy drugs or radionuclide microspheres are delivered directly to the tumor area.
This way, the tumor becomes deprived of blood supply while receiving high-dose targeted treatment.

TACE (Transarterial Chemoembolization): Chemotherapy drug + embolization material.
TARE (Transarterial Radioembolization): Radiation is applied using Yttrium-90 microspheres.


How Does the Treatment Process Work?

  1. Evaluation: Tumor type, size, and location are determined through imaging (CT, MRI, PET-CT).

  2. Planning: The appropriate ablation method for the patient is selected.

  3. Application: The procedure is performed under local anesthesia with imaging guidance.

  4. Follow-up: The reduction and destruction of the tumor is monitored through imaging methods.

During this process, patients are typically discharged the same day or the next day.
The recovery process is extremely comfortable and complication rates are very low.
