The Hidden Part of the Iceberg..

The Hidden Part of the Iceberg in Hormonal Health: Hormone Metabolites Test

In fighting breast cancer and protecting breast health, the most powerful and detailed imaging technology available is undoubtedly Breast MRI. In situations where other methods (mammography and ultrasound) sometimes fall short or provide ambiguous results, Breast MRI acts as an "arbiter," revealing the most hidden details within the depths of tissue.

This method, which doesn't contain radiation and works with magnetic field power, is the golden key to diagnosis, especially in our high-risk patients and complex cases.

Why Is Breast MRI Different From Other Methods?

While mammography and ultrasound focus on the anatomic structure of tissue, Breast MRI also examines the functional and vascular (blood vessel) structure of the tissue. Cancerous tissues need more and irregular blood vessels than normal tissues to grow. The contrast agent (medication) given from a vein during MRI scanning "illuminates" these abnormally vascularized areas, allowing us to determine the tumor's location with millimetric precision.

Key Advantages of Breast MRI:

  • Highest Sensitivity: Breast cancer detection rate exceeds 95%.

  • Success in Dense Breast Tissue: In young women or individuals with very dense breast tissue, it easily distinguishes masses that mammography cannot visualize.

  • Multifocal Detection: If there are multiple tumor sites within the breast, it shows their locations and relationships with the greatest clarity.

  • Implant Monitoring: For women with silicone implants, it is unmatched in examining both the integrity of the implant and the breast tissue behind it.

In Which Situations Is Breast MRI Preferred?

Not every patient needs routine MRI scanning. This method is critical in specific clinical scenarios:

  1. Screening High-Risk Patients: For women with strong family history of cancer or those carrying BRCA gene mutations, it is an integral part of the screening program.

  2. Cancer Diagnosis and Surgical Planning: When a tumor is detected, it is used to understand the true extent of disease before surgery and to check if there are other lesions in the opposite breast.

  3. Monitoring Chemotherapy Response: For patients receiving pre-operative chemotherapy (neoadjuvant), it measures tumor shrinkage with the highest precision.

  4. Clarifying Suspicious Findings: When mammography or ultrasound shows findings that are not clearly understood (BI-RADS 0 or 4), it is consulted to make the final decision.

  5. Breast Implant Monitoring: It is the most reliable method to determine if there is a leak (rupture) in the implant.

The Procedure: What to Expect

Breast MRI scanning is a process that requires some patience for our patients but is painless:

  • Timing Is Important: To prevent hormonal effects from compromising image quality, scanning for menstruating women is preferably performed between days 7 and 14 of the menstrual cycle.

  • Position: Unlike other MRI scans, the patient lies face down in breast MRI. The breasts are suspended into a special apparatus called a "coil."

  • Contrast-Enhanced Scanning: To visualize tumor vasculature, "Gadolinium"-based contrast agent is given through an arm vein. This agent doesn't contain radiation and is generally safe.

  • Duration: The procedure takes approximately 20 to 40 minutes. During this time, the patient must remain completely still for image clarity.

Interventional Radiology and MRI Collaboration

As interventional radiologists, we use MRI not only for diagnosis but also for interventional planning. If there is a very small lesion detected on MRI but not visible on ultrasound or mammography, we perform MRI-guided vacuum biopsy or MRI-guided localization to protect patients from large and unnecessary surgeries.

Doctor's Note: Some of our patients may hesitate about MRI due to fear of enclosed spaces (claustrophobia). However, with the wide tunnel design of our modern MRI machines and the support we provide during the procedure, we complete this process comfortably. Remember, the detailed information that MRI provides directly affects your treatment success.

Conclusion: Don't Compromise on Certainty

There should be no room for question marks in your breast health. If you are in a risk group or if other tests have produced confusing results, Breast MRI will give you the clear answer you need. With this technology that doesn't miss even the smallest detail, you can elevate your defense against cancer to the highest level.
