Although the majority of these nodules are benign, over time they can cause enlargement, difficulty swallowing, a feeling of neck pressure, or aesthetic discomfort. What is a Thyroid Nodule?

The thyroid gland is a small but extremely important butterfly-shaped organ located at the front of the neck. This gland produces thyroid hormones (T3 and T4) that regulate metabolism.
Over time, fluid-filled (cystic) or solid structures can form within thyroid tissue. These structures are called thyroid nodules.
Nodules are usually benign but can sometimes disrupt hormone balance or cause visual discomfort.

The most commonly seen symptoms are:

• Swelling or fullness sensation in the neck

• Pressure or difficulty swallowing in the throat

• Voice hoarseness or sensation of foreign body in the throat

• Palpitations, sweating, weight loss related to hormone imbalance

How are Thyroid Nodules Diagnosed?

At Dr. Atasoy Clinic, thyroid nodules are evaluated with advanced imaging and laboratory tests.

• Ultrasonography: The nodule's size, structure (solid, cystic, presence of microcalcifications), and vascular structure are analyzed.

• Fine Needle Aspiration Biopsy (FNAB): When necessary, samples are taken from the nodule for pathological examination.

• Thyroid Function Tests: TSH, T3, and T4 levels are measured to assess the gland's hormonal status.

If these evaluations show that the nodule is benign, non-surgical treatment methods are initiated.

Non-Surgical Thyroid Nodule Treatment Methods

Dr. Atasoy Clinic has three main non-surgical treatment methods for thyroid nodules:

1. Radiofrequency Ablation (RFA)

Radiofrequency ablation is considered the gold standard in thyroid nodule treatment.
In this technique, with ultrasound guidance, a fine needle is inserted into the nodule. Radiofrequency energy heats the nodule tissue, causing controlled reduction.
The nodule gradually shrinks over several months, and compression symptoms disappear.

Advantages:

• General anesthesia is not required, only local anesthesia is used.

• The procedure takes 20-30 minutes.

• No incisions, stitches, or scars in the neck.

• Patient can return home the same day.

• Vocal cords and surrounding tissues are preserved.

Scientific studies show that RFA achieves 70-90% reduction in nodule volume.

2. Microwave Ablation (MWA)

Similar to RFA, nodule tissue is heated to high frequencies with microwave energy and destroyed.
This method is particularly preferred for large or very solid nodules.
Heat distribution is more homogeneous, so the procedure time is shorter.
Microwave ablation is also a completely non-surgical, scar-free, and painless treatment option.

3. Ethanol Ablation (PEI)

Most effective method for fluid-filled cystic nodules.
Under ultrasound guidance, the fluid inside the nodule is drained, and then sterile ethanol (alcohol) is injected to cause the nodule wall to shrink.
With this method, the nodule does not accumulate fluid again and gradually shrinks completely.

Advantages:

• Takes only 10-15 minutes.

• Results are immediate.

• Only local anesthesia is needed.

• Does not cause irreversible damage.

How Does the Treatment Process Progress?

1. Evaluation: The patient's ultrasound and biopsy results are reviewed.

2. Planning: The appropriate method is determined based on nodule size, content, and location.

3. Procedure: Ablation is performed under local anesthesia with ultrasound guidance in a sterile environment.

4. Monitoring: The patient is observed for several hours and discharged the same day.

5. Follow-up: Nodule volume reduction is monitored at follow-ups at 1, 3, and 6 months.

Who is it Suitable For?

Non-surgical thyroid nodule treatment is ideal for these groups:

• Patients who have been diagnosed with benign (benign) nodules on biopsy

• Those experiencing neck swelling, pressure sensation, or difficulty swallowing

• Patients with high surgical risk or those who do not wish to be operated on

• Those who do not want the entire thyroid gland removed

However, if the nodule is determined to be malignant, surgical treatment is considered.
For this reason, biopsy and detailed ultrasound evaluation are mandatory before any procedure in all patients.

Post-Treatment Period

After the procedure, there may be mild pain or tightness sensation, but this passes in a short time.
Patients can return to work or social life the same day.
The reduction in nodule size is usually noticed from the 1st month, reaching maximum level by the 6th month.
During follow-ups, nodule volume reduction is tracked through ultrasound measurement.
